Registration Requirements

This information must be in-hand to start the registration process

The following information is required to enroll your son/daughter at Oviedo High School. This information must be in hand to start the registration process. Once the initial paperwork has been completed and verified, the student’s counselor will create a classroom schedule, and the remaining cumulative records will be requested.

Current School Records: Copy of the school withdrawal form, sealed transcript and current report card. (No faxed transcripts)

Immunization Records: The state of Florida requires that all immunizations be recorded on the FLORIDA CERTIFICATE OF IMMUNIZATION (HRS 680). This form can be obtained from a doctor, medical clinic, or any public health department office.

Health Physicals: First time students to a Florida school must have a physical dated no earlier than 12 months prior to their enrollment. It must be on a county physical form or doctor’s letterhead. The document must be signed, stamped and dated by the doctor.

Required Proof of Residence - Owned:

  1. Copy of the Warranty Deed or Homestead Exemption
  2. Copy of a current power bill or initial order of service (60 day follow up bill with barcode)
  3. One of the following current documents supporting stated address: Florida drivers license, auto registration, or voter registration as evidence that parent(s) live at the stated addres
Required Proof of Residence - Leased:
  1. Executed copy of the current lease agreement.
  2. Copy of a current power bill or initial order of service (60 day follow up bill with barcode)
  3. One of the following current documents supporting stated address: Florida drivers license, auto registration or voter registration as evidence that parent(s) live at the stated address.

Social Security Carda copy is requested.

Birth Certificate: a copy is requested.

Proof of Custody/Guardianship: Court Documented Custody papers will need to be presented if the student does not reside with both natural parents. If a student is residing with the non-custodial parent, legal authorization from the custodial parent must be presented.

If you are residing along with your son/daughter in another family’s home, Seminole County Form #1312 (Families Residing With Other Families) must be completed and notorized. Residency requirements stated above need to be presented at the time of enrollment. Florida driver’s license must reflect the stated address of both parties. The form is available in OHS Guidance Office.

If your son/daughter is NOT living with parent, Seminole County Form #893 (Student Not Residing with Parent) MUST be completed and returned to the Director of Secondary Education located at the Seminole County School Board Office for authorization prior to registration. The residency requirements stated above must to be presented at the time of enrollment. The form is available in the OHS Guidance Office.
